Kent and Medway Councils trialled an approach to assist people to leave hospital and safely return to their own homes. It involved a team of ‘Technology Facilitators’ providing information, advice and technology to those leaving all local hospitals. We were commissioned to evaluate how the programme ran, what difference it made for those leaving hospital and those caring for them. Based on our findings and in-depth exploration with Facilitators and those using the service we made recommendations on what future iterations of the service should consider.
